What is Case Management?

Case management is a specialized area of healthcare service that focuses on a collaborative, or interdisciplinary, approach to medical treatment.  Case managers assess client needs, create a plan, implement the plan, coordinate interventions and services, monitor progress, and evaluate outcomes - often simultaneously. Case manager clients, or stakeholders, include healthcare users, support systems, treatment providers, employers, and payor sources. It is our opinion that all stakeholders benefit when clients operate at their highest achievable level of health, functional capability, and autonomy.

When should I consider using Case Management?

Case Managers are able to reduce the cost of medical treatment by providing a direct line of communication between stakeholders. They can assist in negotiating prices and discussing alternative solutions to avoid unnecessary interventions and expedite treatment to avoid unnecessary delays that result in poor long-term outcomes and excessive lost work hours. We use national guidelines to advise and educate stakeholders to improve long-term heatlh outcomes and mitigate risks.

Listed below are a few examples of cases that could benefit from case management:

  • Head/neck/spine trauma

  • Fall from height

  • Motor vehicle accident

  • Crush injury

  • Traumatic amputation

  • Complex medical history

  • Previous injury to affected body part

  • Return to work delay/time loss

  • Major joint injuries

  • Injuries requiring surgical intervention

  • Burns

  • Unwitnessed injuries

  • Breakdown in interdisciplinary communication

Early intervention allows the case manager to make the most impact on a case, however, referrals can be made at any point during a course of medical treatment. If you aren't sure if a case is appropriate for case management services, please contact us for more information.

How does it work?

Atwell & Associates provides field case management, telephonic case management, and medical record review services. We accept all case types but specialize in worker' s compensation.

Field Case Management: the case manager meets with stakeholders in person, normally by attending medical appointments with healthcare users and their medical providers, to enable the coordination of interventions and allow open communication with stakeholders. This is our most comprehensive service.

Telephonic Case Management: the case manager communicates with stakeholders telephonically and electronically, at times attending scheduled appointments via audio or video call. Telephonic case management is somewhat less effective than field case management due to lost time while waiting for return calls or needed documentation. The goals are the same as in field case management, but additional turnaround time should be anticipated.

Medical Record Review: the case manager obtains, organizes, and reviews medical records from a specified period of time or related to a specific injury or condition. With this service, Atwell & Associates provide a summary that correlates with the chronologically organized medical record.
